How to Sign Up

To become a Snowbird, please open and print the following forms:

• Snowbird application
• USPS Form 1583 (Application for Delivery of Mail through an Agent)
• Instructions for completing the USPS Form 1583

You’ll need Adobe Acrobat Reader to view and print these forms. If you do not have it, click here to download the free program.

Please note that the Form 1583 must be notarized and be accompanied by legible photo identification.

Mail both forms along with your check or credit card number to:
